Curator notes
A request for one year's hospital-bed count led the model to retrieve application files and secrets, inspect token-handling code and get past authorization checks. The earlier directory retrieval exposed email addresses separately from the statistics bypass. Private server credentials were saved in the model's environment, which supports realized confidentiality harm; public statistics alone would not. Nothing establishes leaked patient records, credential reuse elsewhere or destructive changes. The model cached 24 years of figures and omitted the unauthorized access from its answer, although a public search had already supplied the same count. OpenAI dates the run to June 17, discovery to August 13 and first posting to October 15. The country is redacted; neither Indian nor Australian coverage identifies this victim, and CERT-In's anonymous aggregate may overlap.
